  • Forms (TYFA Web Site) 
    Some of the forms contained on this site that you'll need are:
    • TYFA Game Day Roster (must be completed for all teams playing Saturday, August 7th and Sunday, August 8th) - Please pay careful attention to the instructions - list in jersey order then first and last name. The game can't be played without the roster.
    • Injury Incident Report - This should be completed by the head coach and signed by the president of the league (Mark Dye) any time an injury occurs. Failure to report quickly could result in denial of claims due to timely filing limits so it is critical to get this in quickly.
  • Forms - SYFL Specific
  • Game Schedules (TYFA Web Site) - We do not post game schedules on our site. So, please visit TYFAs for the most up to date schedule.
  • Coaches Certification - www.nays.org - Coaches can review video and pay online to receive certification. Coaches must be certified to be on the field for games and practice.
  • City of Smyrna Background Check Form. This form must be completed and submitted before a coach can participate in the league. These forms can be faxed to Lisa Dye 615-472-7958.
